Driver Records Authorization

I authorize TessyBoyz Tesla Rentals and its designated agents to obtain and review my driving record (Motor Vehicle Report / MVR), including driving history, license status, accidents, violations, and any related information, from the appropriate state Department of Motor Vehicles or comparable agency, and from any consumer reporting agency. I understand this report will be used solely to evaluate my eligibility to rent and operate a vehicle from TessyBoyz Tesla Rentals. I certify that the information I have provided (including name, date of birth, address, and driver's license details) is true and accurate. I acknowledge that providing false information may result in denial of rental, termination of an active rental, and/or legal action. This authorization remains valid for the duration of any current or future rental relationship with TessyBoyz Tesla Rentals unless I revoke it in writing.

Driver Liability & Insurance Waiver

7K Auto LLC dba TessyBoyz

DRIVER LIABILITY & INSURANCE Renter acknowledges and agrees that they are fully responsible for the operation, possession, and use of the rented vehicle during the rental term. Renter shall maintain valid automobile insurance coverage sufficient to cover liability, collision, property damage, and bodily injury arising from use of the vehicle. Renter assumes full financial responsibility for any accident, damage, theft, towing, tickets, or losses arising from use of the vehicle. Under no circumstances shall 7K Auto LLC dba TessyBoyz be held liable for accidents, injuries, property damage, or losses arising from renter's use of the vehicle. Payment of invoice serves as signature and acknowledgment.
